Thank you for your interest in the AmeCon 2008 Masquerade. If you would like to take part, please complete the following form. All fields are mandatory with the exception of group members. Please note that some fields require no more than a certain number of characters to be valid.

Please state the name of all group members in your performance.

Please state the series of the cosplays you'll be using in the masquerade.
